Your first visit to Delaware Family Medicine establishes a vital foundation for our relationship with you. During your first visit, we make sure to obtain important background information about your medical history and give you time to get to know your doctor.

To understand what to expect for your first visit to our practice, please read through our Office Info section. There, you'll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more.     What To Expect

    Being well-prepared for your appointment will ensure that the doctor has all necessary information to provide you the best possible care. It also will help relieve any unnecessary anxiety you may be feeling. 

    Please see our FAQ section for commonly asked questions. We would appreciate you bringing old records, or we can have you fill out a form to request them to the offices you've been before and then upload to your chart. Also, please be prepared to discuss, and if you are able, to bring a paper or digital copy of any or all of the following (concerns, medications, specialists, recent blood work, imaging):

     - a list of your concerns, especially your top 3 in order of what’s most important 

    - a list or the physical bottles of medications including their dosage and frequency (example: acetaminophen 500mg twice a day). If other specialists fill certain medications, please also note this. At your visit, we will discuss if we are able to refill your medications, and if so, which ones.

    *If there are controlled medicines that you request us to refill, please bring the bottle of medication or an official record from a previous doctor. This will be discussed at your appointment if we are able to refill it or would need to return for another visit after gathering testing or seeing a specialist.

    - the names of specialists, including when you last saw them and upcoming appointments 

    - results of blood work within the past year or the most recent labs

    - relevant written imaging reports, such as X-ray, CT, or MRI (physical discs are not able to be read in our office)

    - anything else that you think is relevant and will allow us to care for you smoothly and better understand your goals
